Telehealth Physical Therapist at Volare Health, LLC

Primarily supporting Keizer Nursing & Rehabilitation Center (Keizer, OR) and French Prairie Nursing & Rehabilitation Center (Woodburn, OR)

Compensation: $45–$55/hr (DOE)
Schedule: PRN
Work Location: Remote

• Provide physical therapy services via telehealth to residents in our Oregon facilities

• Conduct evaluations and develop individualized treatment plans

• Guide residents through exercises, functional training, and pain management strategies virtually

• Educate residents and families on safe use of equipment and exercise techniques

• Document patient progress accurately and on time

• Collaborate remotely with on-site nursing, therapy, and interdisciplinary care teams

• Active Physical Therapist license in Oregon (required)

• Graduate of a CAPTE Accredited Physical Therapy Education Program

• Experience in post-acute care or similar healthcare environment preferred

• Strong communication and collaboration skills with a focus on patient care

• Ability to develop and implement effective treatment plans to meet patient goals

• Completes documentation and billing accurately and on time

• Problem-solving skills and willingness to meet productivity benchmarks
